This report has eight sections that includes topics related to the older adult population, the social determinants of health, general health, mental health, lifestyle and health behaviours, social supports and connections, falls, and utilization of health care services. —

This interactive report will allow users to explore the indicators reported here in greater depth. So both individuals and organizations in Wellington, Dufferin and Guelph are up-to-date with the information they need about the well-being of older adults in our region.

The social determinants of health are the socioeconomic factors, within the broader determinants of health, that relate to a person’s status in the socioeconomic hierarchy, such as relative income level, educational attainment, and employment status. These factors encompass the conditions in which people are born, grow, live, play, work, and age, which influence an individual’s access to good health (1,2). These conditions are shaped by the distribution of power and resources at global, national, and local levels, which are themselves influenced by political systems (1,2).The purpose of the Social Determinants of Health Status Report is to provide a view of selected social determinants of health in our communities using data from the 2016 Census issued by Statistics Canada. Indicators for each of the social determinants of health were examined at the municipal level for the Wellington-Dufferin-Guelph (WDG) sub-region.

The interactive nature of the report allows users to explore indicators in greater depth in relation to their communities.

Appendix A: Community Profiles provides an interactive look into the social determinants of health at the sub-region level. This tool has been designed to allow quick comparisons between different social determinant of health indicators.
